Patient services

Careology has dedicated and focused patient coordinators who are knowledgeable about the disease and the patients that they look after.

Prior to the patient being enrolled onto the Careology homecare service a formal registration procedure is followed which involves the completion of a registration form and the receipt of express patient consent.

All patient details are securely held within the Careology patient management system which tracks and monitors individual patient requirements.

A service users guide is provided to each patient and contains comprehensive information about the service with a full list of contact details and details of the official complaints procedure.
 

Patients services coordinators are responsible for:

• Contacting patients following registration with Careology
• Liasing with referral centres and NHS Trusts with regards to prescriptions and other service requirements
• Coordinating deliveries of prescription only medicines and ancillaries and collection of clinical and sharps waste from patients home.
• Rescheduling deliveries to patients if they are unavailable to receive a scheduled delivery
• Liasing with patients regarding nursing visits
• Managing individual patient service requirements such as requests for extra ancillary items
• Holiday and travel assistance management
